24/7 Care in a Home-Like Environment

Our Adult Family Care Residence (AFCR) model, also known as Shared Living, provides around-the-clock support in a certified caregiver’s home. This option is ideal for individuals who can no longer live safely alone and require continuous supervision and daily assistance.

Unlike institutional settings, AFCR offers a family-style environment where participants live in a private residence and receive personalized support.

24/7 Supervision and Support

Personal Care Services

• Bathing and grooming
• Dressing
• Toileting support
• Feeding assistance
• Mobility support
• Medication reminders per care plan

Meals and Nutrition

• Three daily home-cooked meals

• Special diet accommodations when required

Community Integration

• Social and recreational activities
• Community outings
• Religious or cultural participation (if desired)
• Family involvement and communication

Health Coordination

• Appointment scheduling
• Transportation to appointments
• Communication with case managers
• Ongoing coordination with family members

• Furnished bedroom

Our AFCR homes are available to Medicaid participants as well as private pay individuals seeking a safe, family-style living environment.
